Referral details to prepare
- Participant name, date of birth, contact number and email.
- Interpreter requirement and current living situation.
- Aboriginal or Torres Strait Islander identity if the participant chooses to disclose.
- NDIS number, plan management type and primary disability.
- Services requested: Support Coordination, Specialist Support Coordination, Psychosocial Recovery Coach, Positive Behaviour Support, Assistance with Daily Living, Social and Community Participation, SIL or respite.
- Support coordinator details, urgent safety concerns, preferred start date and guardian / nominee details.
